I'm probably in a very small minority or totally on my own with my opinion on this...for me personally, this is way too intimate of a service to be deemed real work or taxable. Might sound crazy but that's what I've always believed, I've never changed my opinion on it since I started doing it, and still don't feel different. There are probably others who feel the same but I doubt they would speak up. I think of the compensation as more of a gift or donation or a trade, and it's up to the recipient if they want to declare taxes on it.
This service is unlike any other, what other type of work involves having your most private body parts being penetrated, with skin to skin contact and exchange of fluids (& mutual pleasure...mutual most of the time hopefully

) & in addition to putting on a performance of acting like a girlfriend and great hostess etc (and now therapist according to the OP)? And prostate exams and pap tests don't count, patients don't get paid for that (unless it's a research experiment)...but seriously? maybe I'm wrong but please enlighten me
