IMHO it's the higher volume providers who need to keep to a schedule and therefore are mindful of the time. They will expect the exchange of funds once you are in the door and then motion you toward the shower. This is typical of touring SP's who may see more clients than the average local SP in one day as a way of covering all their expenses and maximizing their profit.
The lower volume providers that I tend to visit are not real sticklers about the time issue. A short get reacquainted conversation is pretty much off the clock and the appt really starts once I am out of the shower. I keep my showers short since I always shower before traveling to their incalls. I book longer sessions so it's usually me who asks what the time is so I can get back to my real job :nod:
