Why no DFK?

Do certain SPs decline doing DFK because it is a health risk, or because they don't want the session to be brought to the level of intimacy that DFK brings with it? I did a search of perb and did not really come to any clear answer to this. If it is a health risk, what are the potential afflictions that come with it?
