Typically, you discuss the details on the phone. Exchanging the gratuity gets worked into the story, such as when the delivery guy is dropping off the "package". Btw, I have not done role plays with a new visitor that I've never met before. I have to be comfortable with the individual and experience some trust in him and in the scenario.

Yes, there has to be a little acting skill on both sides. I read a review once when the guy said that it was a bit off-putting for him when the lady wouldn't or couldn't 'stay in character'.
