Judging by the hair and suit, I'm guessing that it's a publicity shot from 9 1/2 Weeks. But I was under the impression (perhaps mistaken) that his visage has been affected by years of boxing and hard living -- I was not aware that it was from surgery.
IMHO Mickey Rourke is a much better actor now. He was more of a third tier leading man while now he provides great characters. Man on Fire and The Wrestler come to mind.
Its bad enough that they do too much plastic surgery, but why do they pluck their eyebrows like women? That's what makes them look extra freaky.

According to that, the "before" pic is from 1992, which might make it a promo shot from White Sands then. I'm not sure but I thought he might have already been boxing by then. I did read in that huffingtonpost article that some of the surgery was to repair damage done during his boxing career.
I'm amazed at how some boxers still end up looking so pretty.
