It really depends on what sort of impression you are hoping to make, and the reason for the gift. I agree that generic gifts like gift certificates to spas, chocolates, perfume, flowers, wine etc. are always received well....however, they do run the risk of seeming generic.
If you know her well enough get her something which shows that you have been paying attention - books or music in the genre she appreciates, tickets to a concert you know she would love, a donation to her favourite charity, a membership to something she will appreciate etc.
In all honesty, I think anything which is given from the heart is going to be wonderfully recieved.

A surprise gift to an sp vs a gf are two completely different things, no? Not saying that you are, but they shouldn't be confused as the same thing. I would say buying almost anything for an sp (jewelry, spa, etc) would be easy as they appreciate the gesture. Girlfriends on the other hand would analyze it more..so that means the gift has to mean something..which makes it harder to find the gift. But if u find the right one, it could be inexpensive and she would love u more than ever.
