Ever had a client claim the condom "accidently" slipped off?

I have had four accidents in my life with SP's.

The issues were always combinations of

1) Condom did not fit. This means it went over the tip and about 3inches down the shaft leaving more than 50% uncovered.
2) Not enough lube which caused it to break inside her.
3) Too much lube combined with Erectile Dysfunction.
