It is probably a fake nipple connected to reservoir by her hips where the right hand is every time. Sorry to disagree, but I know from past experience that breast milk does come out in a stream like that when it is expressed like she was doing. I know, I have seen it happen many times when my children were being breast fed.

I'm not so sure breast milk would taste that good in coffee. It's sometime sour, sometimes bitter and more often than not, not sweet enough.
