CIM Etiquette and Positions

Not sure if this was posted before but couldn't really find a similar thread. What are good CIM (Cim in Mouth) etiquette? Obviously make sure to tell the provider ahead of time and letting them know when you're ready to cum. Anything else that are common practice?

And what are some good positions for CIM and what positions do you prefer?
