Is it uncomfortable or inconvenient to remove and put back lip studs or nipple rings, or is it painless and easy? Would it be out of line to ask a provider to take out body or facial jewelry for a session?

I don't believe it is out of line to ask your SP just about anything.
However, if the jewelry is quite obvious in her pics and you don't approve or aren't down with it, then maybe consider someone else?

It depends on long the piercing has been there and if the scar tissue is well healed. If you're asking about traditional earrings or a well-established 3yo lip piercing, it should be fine to pull and replace that for a couple hours.

You will have no luck asking for someone to pull a dermal or surface piercing, no matter how long they've been healing over.
