i agree with thick-one on can america shirts. i was always pleased with the materials, results and the fit. i even used to take off-the-shelf shirts to them to get altered to fit me

I've been going to Blair Shapera, Custom Clothier, for 20+ years now.
He still caters to professional athletes from both the NBA and NHL. I would say there's a lot of credibility there alone.
His steady clientele are from BC judicial arena (Judges and Lawyers).

I know this doesn't help you, but I wait until I'm in Hong Kong and get my shirts made there... (or sometimes Thailand).
If you buy a bundle of them, it works out pretty well. I know there is a Hong Kong Tailor sometimes in town taking measurements (Samtani?), but I think their prices were getting a little closer to local prices.
