Can an infected SP with either Gonorrhea or Chlamydia transmit these STD's by way of BBBJ to a client? How high are the risks? Any info would be appreciated. Thanks!

A quick answer is that gonorrhea and chlamydia can be transmitted by BBBJ. It is hard to work out an exact risk as there are so many variables e.g. does the other person have the infection in the throat, how many partners do you have sex with etc.

We would say that the risk is high because there is a potential for transmission (gonorrhea and chlamydia can live in the throat) and there is evidence of transmission (we have see people get infected this way).

There have been a number of research studies that have shown that chlamydia can survive in the throat. It is unknown how long it will stay there and since we do not have any chlamydia tests for the throat we do not know how common it is in BC.

Could it be that the health nurse means that if someone is positive for the sti's that the risk is high? I am assuming this is what is meant by that statement.

I am glad that you brought this up as I think it will clarify things.

What I meant is that if someone has an STI in their mouth there is a high risk of passing it on. BBBJ is only high risk for certain infections if the person has an STI in their mouth.

The problem you have is that it is hard to work out the odds of someone having an STI in their mouth. This is because we do not have a test to check for chlamydia in the throat and most doctors do not check for gonorrhea in the throat.

Are there any statistics available for the prevalence of either sti received from oral sex only?

Are there any statistics .. percentages of people who walk in to the clinic, who are asymptomatic, who test positive for orally derived stis?

I have received the information about the numbers of gonorrhea and chlamydia infections in BC for 2008.

It is reported in terms of gender and sexual orientation. E.g. male, female, heterosexual, homosexual, bisexual and unknown.

From the data it looks like testing for chlamydia is mainly being performed on the urethra (pee tube) in males and the cervix in females.

With gonorrhea the testing seems to be the same with majority of positive results coming from the male urethra or the female cervix.

The numbers indicate to me that most health care people are not testing the throat or rectum (bum).

There is no information in regard to if the person had symptom or not when they went for testing or what type of risk they had e.g. oral sex only. We just know from our own clinics or other studies what percentage of people would not have symptoms for gonorrhea or chlamydia.


Final numbers 2008 for BC

Chlamydia:
Male: 3638 cases
Female: 6954 cases

Gonorrhea:
Male: 891 cases
Female: 531 cases

I agree that it would be really nice to get a breakdown of the numbers. The only accurate information we do have is gender and sexual orientation. Even a lot of this information has missing information as you can see. We can only use the data that is given to us.

They do have some information about the persons risk e.g. men who have sex with men, customer of sex worker etc. I have been told that since there is so much missing data (most people do not write down the risk) they cannot report accurately on this. They are looking at this for the future to see how they can get this information.

The situation is different for syphilis and HIV as we do follow up interviews with each person and have a better idea of what the risks are.

It does show how people (clients and health care providers) have a hard time discussing sex. I personally think the data is missing because health care providers are not asking the questions and clients feel uncomfortable about talking about their situation.

As discussed above with Max it seems that the main problem is with the missing data. It is great that you feel comfortable giving the information as it really helps.

The data that is collected gets analyzed and gives us an idea of what is going on and where to put resources. It is often not reported out as it is difficult to report strong conclusions when you don’t have the whole picture.
